Whether it’s a routine visit or a hospital follow up, we are providing you and your family an in-house doctor that knows your medical needs.

Membership Pricing
Employer Package
Pricing is dependent on the number of employees and chosen level of participation. Please fill out the membership form and we will be in touch as soon as possible.
Single Adult
18-30 years: $50/month
31-49 years: $75/month
50+ years: $120/month
Married Couple
30 years and under: $80/month/couple
31-49 years: $125/month/couple
50+ years: $200/month/couple
Bonus pricing will be manually changed once registration is complete.
Family Plan
Children ages *2-24 can be added to the family plan for $10 a month.
Single Service
Physicals
One time per service pricing is provided for personal and employer physicals. If you are a Minney Health member the physical is included and any additional investigative studies (i.e. EKG, Stress Test, X-Ray) needed will be at wholesale cost. Please fill out the physical request form on the physicals page and we will be in touch via email. Alternatively, give us a call and we can discuss how Minney Health can best serve you.

What You Need to Know About Your Appointment
Once you submit a membership request, we will reach out as soon as possible to discuss any questions you may have. Once you are ready to join the family we will send you a link for registration. You will need the following items to fill out your registration form.
A List of Your Medicines
The name of the medicine and daily intake.
Medical History (Family and Personal)
This helps the Doctor get a clearer understanding before your visit.
A Debit or Credit Card For Registration
If you are signing up more than one member of your family, our team will manually discount that after registration is complete.
