DPC Membership

Direct Primary Care is a membership plan that offers you direct care without using your insurance provider. Dr. Minney provides most services in-house and can provide prescriptions at wholesale cost. This also includes labs, x-rays, imaging, and pathology results at wholesale cost.

Membership is NOT insurance, and patients are strongly recommended to have insurance to cover specialty costs and catastrophic fees for emergency room and hospitalizations.

Membership Pricing

Employer Package

Pricing is dependent on the number of employees and chosen level of participation. Please fill out the membership form and we will be in touch as soon as possible.

Single Adult

18-30 years: $50/month

31-49 years: $75/month

50+ years: $120/month

Married Couple

30 years and under: $80/month/couple

31-49 years: $125/month/couple

50+ years: $200/month/couple

Bonus pricing will be manually changed once registration is complete.

Family Plan

Children ages *2-24 can be added to the family plan for $10 a month.

Single Service

While we do realize that there are times when nonmembers would like an acute care visit, Minney Health is based on continuity of care and doctor-patient relationships. If an out of town family member of one of our current DPC members need care for an acute illness, we would be happy to accommodate them for a one time fee of $150.

Physicals

One time per service pricing is provided for personal and employer physicals. If you are a Minney Health member the physical is included and any additional investigative studies (i.e. EKG, Stress Test, X-Ray) needed will be at wholesale cost. Please fill out the physical request form on the physicals page and we will be in touch via email. Alternatively, give us a call and we can discuss how Minney Health can best serve you.

What You Need to Know About Your Appointment

Once you submit a membership request, we will reach out as soon as possible to discuss any questions you may have. Once you are ready to join the family we will send you a link for registration. You will need the following items to fill out your registration form.

A List of Your Medicines

The name of the medicine and daily intake.

Medical History (Family and Personal)

This helps the Doctor get a clearer understanding before your visit.

A Debit or Credit Card For Registration

If you are signing up more than one member of your family, our team will manually discount that after registration is complete.
